Payment Policies
Fees
All fees are due and payable at the time services are rendered. In addition, deposits are required on specially prescribed items, including custom inserts and special order shoes. J.T. Murdoch Shoes accepts payment by cash, personal check, MasterCard,Visa, Discover, and American Express.
Insurance
J.T. Murdoch Shoes will accept assignment when the necessary information is supplied to us and we have written/faxed the clients insurance company for prior authorization.
Medicare
Coverage for prescription footwear is extremely limited. J.T. Murdoch Shoes accepts assignment from Medicare, we will submit claims when all the necessary information is supplied.
Medical Assistance
Title 19: J.T. Murdoch Shoes is a provider for Title 19 medical assistance. Most prescription footwear is covered with prior authorization
